Alert mail don't working

My alert email didn’t work for a while, but I send the test mail is successful.
And I can see the Alert in the Notification and Notification History, but it didn’t appear in the event log.

Have somebody get the same problem?
I don’t know what happens and have someone help me?

I checked the maillog, and the file is empty.
I had tried to send the test mail successfully.
Have somebody to help me to resolve it ?

This is my “Default Alert Template” Script

I find the event log has a lot of records (it’s close to 1 million), so I clear the event log on the database.
So, I can receive the alert mail successfully now.
