i have been looking for a couple hours and still trying to fix it. Hoping someone can give us some insight.

see Service alerts cause “Rule created, no errors found” history entries · Issue #13424 · librenms/librenms

Created an everything dynamic group and set filtering on the alerts that had no filtering (all devices).
Sounds like an old bug might have been re-introduced.
